Fiscal Year 2011 Performance Outcome Report

HHS 2013 funding applications pre-qualification process closed on December 30, 2011. We are pleased with a very successful turnout. We received and reviewed applications from 21 agencies, including 6 new, for admission to the FY 2013 HHS RFP process. Detailed summary.

Our Department
The Board of County Commissioners funds the St. Johns County Health and Human Services Department, which offers social support and assistance to needy members of the community.
Our Divisions
The Family Integrity Program, which provides foster and adoption services through a contract with the Florida Department of Children and Families.
The Social Services (Community Based Care Division) assists clients by connecting them with a wide variety of resources to promote, protect, maintain and improve the health status of citizens.
Health and Human Services Advisory Council
The volunteer Health and Human Services Advisory Council advises the Board of County Commissioners on health, mental health, substance abuse and social matters. They also review funding applications for local independent agencies that provide services to St. Johns County residents and make recommendations to the County Commission.
St. Johns County Health Department
Additionally, the County Commission supports the St. Johns County Health Department, an agency of the Florida Department of Health. The Health Department provides a wide array of services including certain Medical Services, Health Education, Public Health Mobile Center, Child Dental Services, Nutrition Counseling, Environmental Health, and Children's Services.
Mental Health Services
Please Note: St. Johns County Mental Health Division transitioned operations to a new provider, Putnam-St. Johns Behavioral Healthcare (PSJBH), effective August 1, 2011. Mental health and substance abuse services continue to be provided at the same location, 1955 US Highway 1 S, in St. Augustine. We are making every effort to provide a seamless transition without major disruption of services. We encourage clients to contact the new provider directly to schedule appointments or discuss any questions they may have. Putnam-St. Johns Behavioral Healthcare may be reached at (904) 209-6200.
Former clients may request copies of their St. Johns County Mental Health Division medical records by contacting the St. Johns County Health and Human Services Department at (904) 209-6030. Requests may require up to two weeks to fulfill and are subject to a service charge.
