Sample Letter

I advise all veterans who are contemplating filing a claim for hearing loss and tinnitus to request an audiology examination.

Dear ________________,

Your client, John Doe; was seen at our clinic for a hearing evaluation on (date). The case history was positive for progressive hearing loss and tinnitus thought to be due to intense noise exposure associated with his/her active military service. Mr. Doe served in the US Navy as a Gunners Mate where his primary responsibility involved the firing of large naval guns. He was frequently exposed to loud environmental noise while involved in gunnery operations with no hearing protection provided.

Pure tone air conduction threshold testing revealed sharply failing bilateral sensor neural hearing losses reaching moderate to severe levels. Speech test results were in good agreement with pure tone findings.

Based on the case history and above test findings it appears that Mr. Doe has hearing losses of a degree and pattern commonly associated with intense noise exposure. It is more likely than not that the hearing loss and tinnitus he experiences is due to his military related intense noise exposure.

There is a visit charge of approximately $100.00 for an audiologist exam and they will provide a letter from the licensed audiologist that can be submitted with your claim. This charge will not be reimbursed by the VA or the Veterans Services Office of St. Johns County.
